Oversees the consolidation, analysis, and follow-up of organization-wide Occurrence Variance Reporting (OVR) and Risk Register development and implementation program. Documents and maintains trending data on patient and staff incidents. Initiates actions for problem resolution with positive outcome. Assists with education of staff and leaders regarding risk management. Takes lead role in implementing EOC Program.<\/p>\n<p><strong>Qualifications:<\/strong><\/p>\n<p>Degree in healthcare field or equivalent qualification by education and\/or experience.Bachelor\u2019s Degree or equivalent<br \/>\nCertified Professional in Healthcare Quality (CPHQ) preferred<br \/>\nCertificate in Healthcare Risk Management preferred.<\/p>\n<p><strong>Responsibilities:<\/strong><\/p>\n<p>\u2022 Supports organization in identifying areas for improvement, setting and implementing local or system wide improvement plans and monitoring the outcomes while updating policies as necessary.<br \/>\n\u2022 Conducts Systematic System Analysis following incidents with harm, potential harm or trends that involves looking at system failures, issuing a report and spreading the learning across health centers.<br \/>\n\u2022 Conducts needed training sessions for staff in identifying and managing their risks and incidents. Support them to demonstrate proactive approach to patient safety and take necessary measures to prevent them from happening.<br \/>\n\u2022 Supports stakeholders in understanding risks in their area of work, develop plans on how to mitigate those risks, and develop shared accountability for implementing and evaluating the plan.<br \/>\n\u2022 Performs risk assessments for new health centers during the planning phase and post-opening.<br \/>\n\u2022 Develops and implements customized Risk Register for each center with stakeholders.<br \/>\n\u2022 Compiles relevant information for the monthly Risk Management Report which consists of an overview with focus on areas for improvement and actionable recommendations.<br \/>\n\u2022 Provides training to all staff on Risk Management System as part of the Quality Improvement Training Program, and the yearly Patient Safety Campaign on the different Quality tools and concepts (PDCA, RCA, Fishbone, FMEA etc.).<br \/>\n\u2022 Serves as an educator, communicator, and resource to staff to understand quality improvement, patient safety, risk management and the organization\u2019s accreditations objectives.<br \/>\n\u2022 Provide required reports to governmental agencies (DOH, DHA etc.).<br \/>\n\u2022 Initiate Quality improvement projects.<br \/>\n\u2022 Conduct EOC and other Internal Mock audits and tracers.<br \/>\n\u2022 Reviews\/Revises quality and risk management programs annually.<br \/>\n\u2022 Maintains an effective system for identifying, evaluating, documenting risk and safety concerns.<br \/>\n\u2022 Assists in coordination of regulatory survey process.<br \/>\n\u2022 Implements HealthPlus wide Culture of Safety Program.<br \/>\n\u2022 Updates and maintains Safety &amp; Risk management Plans regularly.<br \/>\n\u2022 Coordinates the EOC Surveillance Program of HealthPlus. Maintains and disseminates results of the EOC Surveillance.<br \/>\n\u2022 Initiates staff training on completion of patient incident reporting and safety program. Provides orientation and in-service training as needed.<br \/>\n\u2022 Investigates, makes recommendations and provides follow through on all patient incident reports and quality issues.<br \/>\n\u2022 Directs ongoing, organization wide collection of information about deficiencies and opportunities for improvement.<br \/>\n\u2022 Reports on findings, recommendations, actions taken and results of the quality and risk management programs to the appropriate committees.<br \/>\n\u2022 Implementation of JCI standards &amp; preparation for the JCI accreditation.<br \/>\n\u2022 Implements HealthPlus wide Culture of Safety Program.<br \/>\n\u2022 Updates and maintains Safety &amp; Risk management Plans regularly.<\/p>\n<p><strong>Required Experiences:<\/strong><\/p>\n<p>Exhibits knowledge of Risk Management processes and safety initiatives in a healthcare setting. Exhibits knowledge of Quality Management processes. 5 years experience of Risk Management or Quality Management in a healthcare setting. Knowledge and experience with JCIA, preferred. Minimum of two (2) year experience at a supervisory level.<\/p>\n<p><strong>Professional:<\/strong><\/p>\n<p>\u2022 Demonstrate ability to set priorities<br \/>\n\u2022 Ensure accuracy and completeness of work performed. Submit reports , and conclude assigned projects\/ tasks within the deadline<br \/>\n\u2022 Ensures that the department is meeting the Key Performance Indicators based on their Operational Targets<br \/>\n\u2022 Demonstrates flexibility &amp; dependability<br \/>\n\u2022 Accepts and completes other assigned duties and responsibilities<br \/>\n\u2022 Participates in educational activities\/training , which contribute towards professional growth<br \/>\n\u2022 Incorporates knowledge of legal and ethical consideration in job<br \/>\n\u2022 Maintains a professional image of self<br \/>\n\u2022 Arrives to work dressed in a professional manner<br \/>\n\u2022 Approaches situations and day to day activities in a positive manner which avoids antagonism<br \/>\n\u2022 Demonstrates ability to lead groups evident by atmosphere of teamwork, efforts to obtain input of others involved in planned activity and lack of internal conflict<br \/>\n\u2022 Remains calm under stressful situations using logical and sound judgement to choose appropriate course of action as evident by observation<br \/>\n\u2022 Demonstrates effective communication skills. Both verbal an written communications are clear, concise, complete and accurate<br \/>\n\u2022 Wears unobstructed HealthPlus identification badge at all times while on duty to identify self to patients, visitors, physicians and co \u2013 workers<br \/>\n\u2022 Maintains confidentiality of patient and employee information.
